Measurements:

I agree to the following:

1. I verify that the above information given by me is correct.
2. The above measurements are mine and are correct, and I accept a size selection by Bella Bridal Gallery from the designer's sizing Chart. OR I request size
3. I understand that alteration will be needed, and that Bella Bridal Gallery is not responsible for any sizing issues.
4. I will pay EXTRA for my alterations, & I will make the alterations appointment myself.
5. That if I request a dress to be shipped from Bella Bridal to me, I will pay EXTRA.
6. That if for any reason, such as gain/loss of weight, if I request the size, or if I become pregnant, Bella Bridal Gallery will not be responsible for the dress sizing problems.
7. The bride will be the point of contact between Bella Bridal Gallery & me, and all my inquires regarding this order will be communicated through the bride.
8. Orders are not subject to cancellation, and deposits are not refundable.
9. Any merchandise not picked up by the  documented wearing date is the property of Bella Bridal Gallery.
10. Bella Bridal is not responsible for any merchandise damages after pick-up.
11. I will pay $25 charge for retuned checks.
